General sizing chart:

Use the size of your nursing bra as a guide. For example, if you are wearing a nursing bra that is a 34-C, you would wear a Medium in our bras.

If you are the far edge of both the band size and the cup size (i.e. 36-D), you should go to the next size up (in the case of 36-D, you should choose a Large).
